Peer-Reviewed Study · Wolraich ML, Hagan JF Jr, Allan C, et al.; AAP Subcommittee on Children and Adolescents with Attention-Deficit/Hyperactivity Disorder · October 2019

The AAP recommends evidence-based parent training in behavior management and/or behavioral classroom intervention first for preschool-aged children, when available. Methylphenidate may be considered if behavioral interventions do not provide significant improvement and moderate-to-severe functional disturbance continues.

For preschool-aged children (age 4 years to the sixth birthday) with ADHD, the PCC should prescribe evidence-based PTBM and/or behavioral classroom interventions as the first line of treatment, if available.

Key Action Statement 5a

The AAP recommends evidence-based parent training in behavior management and/or behavioral classroom intervention first for preschool-aged children, when available. Methylphenidate may be considered if behavioral interventions do not provide significant improvement and moderate-to-severe functional disturbance continues. The AAP subcommittee used an AHRQ Evidence-based Practice Center review covering 2011–2016, and more than 30 internal and external stakeholder groups participated in peer review. This is not a blanket recommendation to replace all indicated care with generic CBT.
